I tried Vista voice rec for a while for command and control because my hands are in such bad shape. I wanted to reduce my mouse usage because that gave me the most problems. I used it for mouse clicks by saying "left click" and "right click" etc... These mouse clicks are not available in Vista command and control, I had to program it into Vista (I am a programmer by profession). While it is better than Dragon Naturaly speaking for command control, voice rec grows tire some constantly saying left click etc, and you have to have a headset on all day and take it off when you are on the phone. The best solution I found for eliminating using the mouse is Natural Points infrared cam. You basically put a reflector dot on your forhead and the mouse moves with your head. Works extremely well. I also have foot pedals for mouse clicks. This combination has saved my hands.
